Our Services
1915(i) Individual and Transitional Supports (ITS)
Individual Support is a “hands-on” service for persons with Serious and Persistent Mental Illness (SPMI). The intent of the service is to teach and assist individuals in carrying out Instrumental Activities of Daily Living (IADLs), such as preparing meals, managing medicines, grocery shopping and managing money, so they can live independently in the community. The goal is that the need for this service will fade or decrease over time as the individual becomes capable of performing some of these activities more independently.
Outpatient Therapy
This is a service designed to meet the clinically significant behavioral or psychological symptoms or patterns that have been identified as treatment needs of the recipient. This service is provided through scheduled therapeutic treatment sessions. The service may be provided to an individual, couples, families or groups.
Day Supports (Individual & Group)
Day Supports is primarily a group service that provides assistance to the individual with acquisition, retention, or improvement in self-help, socialization and adaptive skills. Day Supports are furnished in a non-residential setting, separate from the home or facility where the individual resides. Day Supports focus on enabling the individual to attain or maintain his or her maximum functional level and is coordinated with any physical, occupational, or speech therapies listed in the Individual Support Plan. Transportation to/from the individual’s home, the day supports facility and travel within the community is included.
Psychosocial Rehabilitation
Psychosocial Rehabilitation (PSR) - is a facility-based service
designed to help adults with psychiatric disabilities increase their
functioning so that they can be successful and satisfied in the
environments of their choice with the least amount of ongoing
professional intervention. PSR focuses on skill and resource
development related to life in the community and to increase the
participant’s ability to live as independently as possible, to manage
their illness and their lives with as little professional intervention as
possible, and to participate in community opportunities related to
functional, social, educational and vocational goals.

Clinical Assessments
Clinical/functional evaluation to determine need for Enhanced Services by Medical Director and/or Licensed Clinicians.
Child and Adolescent Day Treatment
Focuses on enhancing interpersonal relationships, and communications skills, the Day Treatment program builds on strengths and addresses identified functional problems of the complex condition of each child/adolescent and family. Interventions support symptom reduction and/or sustain symptom stability at the lowest possible levels. Services are available to children 5-17 years of age.
Peer Support
Peer support is the “process of giving and receiving encouragement and assistance to achieve long-term recovery.” Peer Support Specialists “offer emotional support, share knowledge, teach skills, provide practical assistance, and connect people with resources, opportunities, communities of support, and other people.” They also offer their unique lived experiences with mental health conditions to provide support focused on advocacy, education, mentoring, and motivation. Advocating for individuals in recovery while sharing resources and building skills are significant components of the Peer Support service delivery.
Medication Management
Medication management is the level of outpatient treatment where the sole service rendered by a qualified physician, or others whose scope of practice includes prescribing medication, is the initial evaluation of the patient's need for psychotropic medications, the provision of a prescription, and, as-needed, ongoing medical monitoring/evaluation related to the patient’s use of the psychotropic medication.
Community Living Facilities and Supports (CLFS)
Community Living Facilities and Supports (CLFS) helps adults with intellectual and/or developmental disabilities who want to live independently in their community. Individuals will receive the following information and supports through CLFS:

Affordable housing options and residential supports, if needed,

Support with navigating their services and supports, including financial help such as Supplementary Security Income (SSI),

Oversight of their healthcare and long-term service needs, and
-
Meaningful day services of their choice.
The individual will need an independent Care Coordinator who can provide them with valuable information and resources. The individual must stay in the home that they own, their family own or have a lease in the community.
Community Living and Supports
Community Living and Supports (CLS) helps individuals with intellectual/developmental disabilities live more independently in their homes and communities. CLS supports community integration, including activities like shopping, banking and social events. CLS offers support and training to help individuals learn (1) new skills, (2) practice or improve existing skills, (3) become active members of their community, and (4) make informed choices. Areas of skill acquisitions are:

Intensive In Home Services
This is a time limited intensive family preservation intervention intended to stabilize the living arrangement, promote re- unification or prevent the utilization of out-of-home placement. These services are delivered primarily to children in their family’s home with a family focus. This intervention uses a team approach and is time limited depending on the presenting stabilization needs of the youth and family.
